HOUSING CASE MANAGER
Full-time case manager position conducting intake of clients requesting rental & utility assistance to assess & evaluate client needs & eligibility for services.  Develops case plans designed to meet client needs & verifies supporting documentation for determining eligibility.  Experience working with diverse populations, inclusive of homeless individuals. Make referrals for clients to various community services and resources & document progress.  Experience with individual counseling is required and conducting group sessions is a plus in areas such as budgeting and credit counseling.

Bachelor’s Degree or four (4) years related work experience.  Knowledgeable of community resources & good communication skills both verbal & written.  Must have proficiency with Microsoft Office & Access database as well as experience working with diverse populations, inclusive of homeless individuals.  Must meet Immigration Law requirements.  $14.32/hr. - $16.03/hr based on experience.

AREA AGENCY ON AGING
Caseworker Protective Services’ duties may include: taking reports for crisis intervention; providing direct services to clients who are at risk or in an emergency situation; periodically reassessing care plans; providing general counseling; and making referrals.  In addition to degree, 1-2 years of experience in crisis services or law enforcement experience preferred.  Experience in investigation resolution of problems in a crisis situation for resolution.  All Caseworker positions require Bachelor’s degree. Budgeted range at $16.04-$20.05 per hour starting, depending on prior experience, with excellent benefit package

Caseworker Waiver Service Coordinators develop service plans and coordinate direct services to participants to assist in the identification and resolution of service needs. Degree which includes or is supplemented by 12 college level credit hours in sociology, social work/social welfare, psychology, gerontology or other of the behavioral sciences and 1-2 years of experience preferred.  All Caseworker positions require Bachelor’s degree. Budgeted range at $16.04-$20.05 per hour starting, depending on prior experience, with excellent benefit package.

RN Consultant/Specialist performs medical review of cases and services.  Consult and counsel clients using your RN expertise not hands on care.  Duties may include: assisting consumers through person centered counseling to help make informed health care choices, working with an interdisciplinary team to identify appropriate services and supports, and visitation or phone contact with consumers. Completion of a two or four year Nursing School Program and valid Pennsylvania RN license is required.  Budgeted salary starting at $41,039 and up depending on depending on prior experience, with excellent benefit package. RN Position includes Regular hours – No more changing schedules or shift work.  

Good communication skills both verbal and written.   Ability to speak or read languages other than English, especially Spanish or Russian, is desirable. Good outreach skills and knowledge of community resources needed. Proficient in the use of the computer and have computer software skills including Microsoft Office software. Must be able to travel.  Must have a valid PA driver’s license with full-time access to private automobile and auto insurance as required by GECAC. 

Caregiver Support Program Secretary performs secretarial/clerical functions for the Aging Division.  Ability to prepare reports and maintain files and records, edit and proofread business documents.  Preference to those able to read and keyboard at least 40 CWPM, Good interpersonal skills to work with individuals both internal and external. Salary range $10.80 - $13.47 depending on prior experience. Must have original Act 34 Clearance issued within in past 5 years.
